Daniel joined Active Day as Chief Operating Officer in January, 2019, drawn by the opportunity to fulfill our important Vison and Mission, understanding the impact this has on the lives of our members and those who care for them. He brings to Active Day a broad perspective and experience with 30 years of operations leadership in senior living, home health, home care and hospice. He has held leadership roles in companies including Life Care Services, Sunrise Senior Living, Addus Home Health, and most recently, held the position of COO of Almost Family. Daniel is committed to achieving excellence in all areas of operations by consistently executing the “Active Day Way” and fulfilling our promise to members, families, team members, referral source and payers alike.

Vice President of Business Development
Len brings years of experience in healthcare finance and business development. In his most recent role, he served as head of business development for Veterinary Practice Partners, where he built an acquisition platform that greatly accelerated the company’s growth. “Acquisitions are an important part of Active Day’s plan for expansion,” stated Bob Creamer, CEO. “We need to ensure that we are adding centers that align with our mission, as well as provide additional opportunities for growth.” Len will work with the Business Development team to build a scalable process for identifying, building relationships and bringing and bringing new centers in the Active Day family.

Vice President, People and Culture
Cat brings over 10 years of Strategic Human Resources leadership to Active Day, most recently leading HR operations for two Operating groups at DaVita. Cat has a passion for leadership development and employee engagement and has created recruiting, onboarding and development programs to enable business operations and growth. With her focus on People and Culture, Cat looks to transform the way we hire, train and develop our leaders to best support the evolving needs of the communities we serve.

National Director Clinical Operations
As the leader of our clinical team, Debbie will focus on providing the clinical support needed to lead us in a number of critical areas, allowing us to distinguish ourselves as a progressive healthcare provider and a model for the industry. Debbie brings a unique mix of nursing and leadership experience and knowledge to the organization. She previously served as the National Clinical Director for Seniorlink and has had a successful career working in multiple home health organizations. Additionally, she was an Adjunct Professor at the University of Connecticut School of Nursing. Debbie’s strong capabilities in home and community based services will drive Active Day to set the bar for clinical quality in the industry

Chief Executive Officer
Bob Creamer joined Active Day as Chief Executive Officer in October 2017. Bob is devoted to growing the reach of Active Day across the country. He brings significant experience leading high growth, multi-state health service organizations. Over the past three decades, Bob has built healthcare delivery models that improve patient outcomes and increase business efficiencies, most notably at Gentiva Health Services. With his guidance, Active Day will continue to raise the profile of adult day health services so that more people can benefit from our services.

Director of Business Development
Howard has been leading Active Day’s Business Development efforts since 2015, during which time the organization has completed the acquisition of 20 centers and expanded into two additional states. In his role, Howard represents Active Day across the country at industry events and in national lobbying and advocacy. He has interacted with hundreds of adult day service centers and companies and brings extensive familiarity with various reimbursement, payor, and business models across multiple states. With a diverse professional background in the healthcare, pharmaceutical, logistics, and financial services industries in both large and small companies, Howard brings a knowledge base that spans financial, operational, human resources, and IT areas. From spending time at our centers and meeting center owners across the country, Howard maintains a deep appreciation for the important role we play in the days and lives of our members. Howard currently serves as a member of the Board of Directors of the National Association of Adult Day Services.

Vice President of Information Technology
Active Day recently established this role so that Irvin can lead major technology initiatives on behalf of our brand. These innovations, which improve day-to-day operations at our hundred plus centers, include integrating a new Electronic Health Record (EHR), accelerating our Practice Management System, and engineering a new reporting and data analytics platform. With 16 years of healthcare technology experience, Irvin cites the satisfying opportunity to serve the communities through an amazing organization as the most exciting aspect of his role. He sees a direct link between introducing new technology concepts and enhancing Active Day’s ability to provide quality care.
